Hi Greg		Most likely you will need to contact your local town or city
hall to obtain a permit.  They will have information and paper work you
need to obtain before you can build.  Most building depts. are very
helpful and willing to work with you.  Different localities adhere to
different guidelines and policies. In addition to your list a state
certified septic design,  some permits require the license # of the
builder, plumber, electrician etc.
