I just last fall installed a (natural gas) Aquastar tankless in my (conventional) remodel, and while turning on the water all the way to hot does decrease the flow (appx. half, I would guess), it is HOT - too hot to use, so when cold is mixed in, the pressure is fine.  Of course, even though the heater has only about 8 ft. (almost 3 meters) of pipe between it and the sink (all insulated), The water looses enough heat in the winter here (Southern Oregon) that I have to turn the adjustment on the heater itself all the way up.  The energy savings, I'm sure, are still quite substantial.  Oh yes, the building dept. here insisted I mount it on the outside of the building, in a well ventilated AND heavily insulated(!) cabinet of it's own.  I still put a 60 watt bulb on a thermostat below the tank in the cabinet just to be sure it doesn't freeze in winter...
